When Do You Need a Headboard? A headboard is needed if the studs do not align with the pre-drilled holes in the rail, or if there is a doorway trim, and the combined "door depth" plus "trim depth" is greater than 2 inches.

To determine the required spacing, calculate: "2 inches - door depth - trim depth". If the result is 0 or greater, then you're fine. If the result is negative, that number is the extra spacing you need, which can be accomplished by bolting a header board to the studs and then mounting the rail to this header board.
